Home Health Aide

Combined Living Care are looking to recruit a Registered Nurse to join us. This position would be suited to an enthusiastic and passionate practitioner who wants to make their mark within the NDIS.

Position

This position is a key member of the Company and is responsible for providing high-quality nursing care and support to meet the changing clinical needs of clients. This position will assist clients in their home environment and provide comprehensive and accurate nursing assessments, undertake case management of clients and apply expert clinical knowledge to care plans.

You will also manage the nursing needs and work with the CEO on the resources required.

Assessment and Care Planning

  • In consultation with clients, participants, carers and families facilitate care assessment, care planning and review
  • Develop and monitor individual plans and budgets with regular reviews/case conferencing
  • Manage any identified risks within principles of Positive Approach to Challenging Behaviours and Duty of Care, Dignity of Risk
    • Development and maintenance of all documentation in client management systems
  • Work within legislated NDIS and CDC frameworks
  • Contribute to quality processes such as management of client feedback

Clinical Practice

Coordinate, supervise and monitor delivery of care by care staff and delegate tasks appropriately.

  • Communicate and liaise with the clients GP and other Allied Health Professionals.
  • Perform all clinical practice in a manner that complies with all relevant legislation, common law, regulatory requirements and professional standards affecting nursing practice.
  • Maintain registration requirements of AHPRA and adhere to all legislative and regulatory requirements.
  • Be aware of and comply with legislation and organisational requirements relating to Workplace Health and Safety and infection control.
  • Demonstrate contemporary nursing knowledge and skills by maintaining practice standards in alignment with professional registration requirements
  • Actively participate in professional development activities (internally and externally) to maintain contemporary nursing knowledge and skills as well as acquire new knowledge and skills

What do I need for the role?

  • Current registration as a Registered Nurse with AHPRA
  • Proven recent experience in the community would be preferred
  • Excellent time management
  • Supervisory skills and ability to lead and train subordinate staff
  • The ability to contribute to and inspire a team in the delivery of person centred care
  • Compassion and a genuine interest in caring, supporting their dignity, choices and independence
  • Some knowledge of NDIS (desirable)

Job Type: Casual with the plan to become permanent

Pay: $40 - $80 per hour
