We have a range of teams and professionals who will help you or your loved one with a disability. Our clinical health and disability services can be funded through the National Disability Insurance Scheme (NDIS), self-managed and plan-managed participants. Assistance with Daily Life

Participants and their families can feel comfortable knowing that our support workers will be alongside them and assisting them while they enjoy this much-needed aspect of their daily lives.

  • Showering
  • Dressing
  • Toileting support
  • Assistance with medication
  • Preparation of food and meals
  • Other light household tasks (vacuuming, mopping, cleaning, dishes, changing bedding and other basic chores around house)
  • Garden Maintenance – Keeping your garden well maintained is important for your comfort and also your safety. We can help with lawn mowing, gardening, gutter cleaning and keeping outdoor spaces swept and free from dirt so you can enjoy your outdoor space and keep it looking its best.

Our support workers are carefully selected to make a positive difference in each individual participant’s day.

We know how important it is for our participants to be included as part of the community and to partake in daily life and socialising. Our support workers are ready to assist with:

  • Visiting friends and meeting new friends
  • Attending a group outing or community event
  • Starting a new hobby
  • Attending a family get together

Transport

Transportation is an important part of everyday life. You may need it to get to work, a place of study, medical appointments, run day-to-day errands and generally access the community. We offer necessary support with transportation which enables participation in the community, social, economic and daily life activities.

We can provide you with safe and comfortable transportation with a well-trained licensed driver or one of our well-trained support workers can accompany you on trains, buses and other forms of transportation to help you get to:

  • School or another educational facility Work
  • Your doctor or other medical appointments
  • Your local shopping centre
  • Around your community

Consumables

Consumables are everyday items you may need because of your disability support needs. For example, continence products or low-cost assistive technology and equipment to improve your independence and/or mobility.

Includes the following but is not limited to:

  • Early Childhood continence consumables
  • Continence Consumables
  • Home Enteral Nutrition
  • Pharmaceuticals

Assistance with Social & Community Participation

Social and recreational activities are part of everyday life. These are activities everybody does for fun and can help with your health and well-being. They can also help you to meet new people and improve your skills. When we talk about support for social and recreational activities funded under the NDIS, we mean the extra help you need to take part in these activities because of your disability.

Social and recreation activities can include things like:

  • Visiting your friends and family
  • Active hobbies, such as bike riding, skiing or kayaking
  • Playing sport, such as tennis, surfing or basketball
  • Going out, for instance to the movies or a concert
  • Going places for fun, such as shopping or visiting a museum
  • Relaxing, like meditation or yoga
  • Learning new skills, like dance, art classes or quilting.

You will need to pay for the costs of the activity that everyone would pay for such as membership or entrance costs. We may then be able to fund the support you need to take part in the activity because of your disability.

You may need short-term support to help you get started with your social and recreational activities. This short-term support may build your skills so you can participate independently. Or, it could connect you with someone else who can regularly help you to participate. You might also need ongoing support to participating in social and recreational activities. If you need ongoing support we may fund this as well.

You may already have funding in the core budget in your plan. You can use this for the help you need to participate in social and recreational activities because of your disability. Check with us if you aren’t sure how to use the funding in your plan for social and recreational supports

Assistive Technology

The NDIS provides people with a permanent and significant disability, their families and carers, assistance to help that person in their daily life and help them increase their independence and participate in the community.

Our products and services give people with a disability the support they need to live safely and independently, at home, at work or in the community.

Home Modifications

Home modifications can be minor changes or they can be complex.

Minor home modifications are home modifications that don’t change the structural parts of the home or cost more than $20,000. For example, widening an internal doorway that isn’t holding up other parts of the home is likely to be a category A minor home modification. 

Complex home modifications are changes involving a few areas in your home. For example, you might need to combine your bathroom and toilet to give you more room to use a hoist or shower chair.

You may need home modifications if you:

  • Have safety concerns or difficulty moving around your home
  • Have trouble with personal care, like using your shower
  • Can’t use the rooms you need to use in your home
  • Have difficulty pursuing your goals in your home because of your disability
  • Need changes to your home so your carers can support you safely. 

In some circumstances, the NDIA may fund reasonable and necessary supports that are related or incidental to home modifications. For example, short-term rent if you need to move out of your home during building works.

As with any building work, there are laws and regulations that need to be adhered to when undertaking home modifications under the Scheme.

Coordination of Supports

Want a provider that gets you? Combined Living Care can connect you with the right NDIS services. 

When you have an NDIS plan, you can decide which service providers you work with. Some people find and coordinate their services themselves, but it can be quite difficult. That’s when Support Coordination might be included in your NDIS plan.

Support Coordinators can help you find mainstream and other disability services that are right for you. There are three levels of support coordination that can be included in your plan. These are support connection, coordination of support, and specialist support coordination.

At Combined Living Care, our Support Coordinators help you make the most of your NDIS funds. Combined Living Care Support Coordinators can help you find and connect with different mainstream services.

This could be a TAFE course, a physiotherapist, or even getting a mental health plan through your GP. They can also help you:

  • Be a part of your community
  • Find and connect with local NDIS supports
  • Understand your NDIS plan
  • Understand your budget
  • Work out the best way to spend your funds
  • Use the myGov website and NDIS Participant Portal
  • Prepare for your NDIS plan review.

Improved Living Arrangements

Depending on your goals and circumstances, we can help you access temporary or permanent housing. Whether you’re self-funded or need help connecting with other funded housing programs, we can help you navigate the process so you have a suitable place to live. 

A comfortable, appropriate living arrangement is essential for a good quality of life. As an NDIS participant, the right housing can help you enjoy living independently and increase the choices and options that are available to you. Individualised housing may be more inclusive than a group home or other supported living arrangement, enabling you to be integrated into your community and access more informal support. 

In order to access funding for this capacity-building support item, you’ll need to include relevant goals in your NDIS plan. Usually, this funding is used to help you find and move into a new living arrangement, so you should include a goal of moving to a new home in your plan. 

Your plan will also need to explain why you need to change your current living arrangement. It should discuss what is lacking in your current housing situation and how a different living arrangement will improve your independence or quality of life.  

If possible, it’s also helpful to include a note from your GP about your housing needs. This can discuss how your disability affects your housing needs and what type of living arrangement would be most appropriate for you

Increased Social and Community Participation

This funding covers participants’ need to meet goals relating to participating in a community, social or recreational activities. These activities may be provided in a centre or in a community setting.

You could use this funding for a Disability Support Worker to support you to travel and attend a community event, such as a music concert. This funding also covers support for activities that are either centre-based or out in the community, such as attending a gardening group.

Other activities can include the following and more:

  • Attend personal development courses
  • Join a social group
  • Take an outing or a holiday
  • Go on a holiday camp
  • Visit your local library
  • See a movie or a concert
  • Go to community events, and/or
  • Join a sporting club

Finding and Keeping a Job

There are many benefits to having a job, and we want to help NDIS participants who want to work, get the job they like and keep it.  Having a job expands social networks, raises living standards and increases financial independence and self-worth. There are a lot of ways that we can help you to pursue your employment goals.

Combined Living Care offers specialist employment assistance to help people with disability, injuries or health conditions, find and retain suitable employment in the open labour market. We can also offer support to your employers.

There are two parts to Disability Employment Services:

  • Disability Management Services - we help job seekers with disability, injury or health conditions, find a job. We can also provide occasional support in the workplace.
  • Employment Support Services - We help job seekers with a permanent disability to find a job if they need regular, ongoing support in the workplace.

Improved Relationships

Improved Relationships is a budget under Capacity Building in an NDIS Plan. The aim of Improved Relationships is to provide funds for specialised assessment and supports where a person has complex or unclear behavioural needs.

Combined Living Care support people on the NDIS to improve communication, behaviours and interactions with the people around them.

Improved Relationships cover two key areas:

  1. Specialist behavioural intervention support: Specialised assessment and supports where an NDIS participant has complex or unclear behavioural needs. Behaviours of concern may include persistent behaviours that limit a person’s ability to have a good life or risk physical safety.
  2. Individual social skills development: Supports that assist people on the NDIS to develop social skills to participate in community or social activity.

The types of supports covered vary depending on individual needs but may include:

  • Specialist behaviour and intervention supports
  • Training for carers in behaviour management strategies
  • Support to develop social skills
  • Intensive behaviour intervention.

Improved Health and Wellbeing

Improved health and well-being supports are capacity-building supports that help build your independence and skills to help you reach your long-term goals. It includes exercise or diet advice to manage the impact of your disability.

Whatever you need to feel good each day, our allied health professionals can work with you to build your independence and improve your physical and emotional wellbeing.

We’ll listen carefully to find out what’s important to you and things you’d like to achieve.

Together, we’ll develop your personal plan which could include getting support for:

  • Diet and nutrition
  • Exercise and staying fit
  • Managing anxiety
  • Language and communication skills
  • Managing everyday tasks including showering, dressing and eating.

You’ll have access to a qualified and experienced team of occupational therapists, psychologists and exercise physiologists who can provide support in an individual and group setting.

Improved Learning

Need help transitioning from school to university, TAFE or other tertiary studies?

We can help you navigate your lifestyle change from school to university or further education by building your capacity and developing your skills so you can live and study more independently.

If you have a current NDIS plan with funding for ‘Improved Learning’, you may be eligible to access a range of our supports. 

Our Improved Learning supports may include helping you to budget on your own, assistance with time management, setting up student accommodation/living arrangements and connecting you with other mainstream support services.

Improved Life Choices

By utilising a Plan Management service for your NDIS package, you increase your capacity to experience the full array of supports available to you. This is achieved by removing any related stress, confusion or time spent on budget planning, administration and payments to support providers.

Your time should be focused on selecting from all the brilliant supports available to you. Improved Life Choices is about experiencing life with minimal interference from the tiresome planning and administration of your NDIS funding. We want you and your family to have peace of mind that the ‘paperwork’ and the ‘numbers’ are all in safe hands, with your best interests at heart.

Assistance in Educational Programs

As part of our Improved Learning supports, we can help you with behavioural support, participation and engagement, personal care and social interaction so you can feel confident integrating with your new classroom.

Social Skills Workshops

We offer both 1-on-1 and group workshops with peers to help you learn positive ways to deal with common feelings like stress and anxiety as you continue your studies and enter the workforce.

Time & Workload Management

Entering higher education can be quite different from high school. Many institutions do not offer the same level of support that many high schools do, which is why we’re here to help you manage your time and workload for a healthy work-life balance.

Guidance & Skills Training

Our friendly support staff can help guide you through the application process with your educational program of choice. We can also help connect you with other supports and providers like student services or disability liaisons so you can study as independently as possible.

Improved Daily Living

We can help you develop, build and maintain the skills to live in the way you wish at home and participate more in your local community. 

If you have a current NDIS plan with approved funding for ‘improved daily living’ under your capacity building supports category, you may be eligible to access a range of our Occupational Therapy, Psychology and Counselling services.

Individual Assessments and Reporting

Many of our customers require the support of a health professional to maintain or improve participation at home or in the community. Our friendly team of Allied Health professionals can help assess and provide a detailed report so you can get access to the right support, funding and equipment for your specific needs and goals.

Therapy Programs

Our team of registered Allied Health professionals, occupational therapists, psychologists , social workers and counsellors can help you strengthen your life skills to achieve your goals through an individualised support program.

They can work with you and your support team to work out a plan to support your goals This could include help with language and communication, interacting with other people, managing anxiety, interacting with your environment and prescribing equipment. We can also provide therapy in individual and group formats

Daily Living and Life Skills

Our team of trained support workers and allied health staff can help you identify strategies to manage your day-to-day life. We can help you learn how to travel more independently, as well as manage other daily tasks like showering, dressing, cleaning, cooking, budgeting, meal planning and more – all depending on your needs.

Care Support and Advice
