Assistance with life stage transition service includes short and long terms support that aims at strengthening and supporting the participant’s abilities and choices. Transitions such as moving out of one’s home, joining the university, or getting a job are part of everyone’s life. It is never an easy task both emotionally and practically, and it becomes a little more difficult for people with disabilities. Change can be hard and with it comes new environments and settings. The idea is to make this transition phase as smooth as possible without disrupting the comfort of the participant. We work with the participant to enable a friction-free move and encourage the participant to settle in a new setting and create a comfortable attitude. Some our services for this purpose include:

Therapy Programs

They can work with you and your support team to work out a plan to support your goals This could include help with language and communication, interacting with other people, managing anxiety, interacting with your environment and prescribing equipment. We can also provide therapy in individual and group formats

Our team of trained support workers and allied health staff can help you identify strategies to manage your day-to-day life. We can help you learn how to travel more independently, as well as manage other daily tasks like showering, dressing, cleaning, cooking, budgeting, meal planning and more – all depending on your needs.

If you’re a primary caregiver for a person with a disability, our specially trained staff can help support you in your duties and offer advice alongside our plans and assessments to ensure you feel confident and supported in your role as a carer.
